There are "system-wide" problems in the way the NHS investigates patient deaths which leave family members in the dark about failures in care, a new review has found.
Too often grieving relatives were shut out of investigations or left without clear answers, the Care Quality Commission (CQC) said.

South London and Maudsley NHS Foundation Trust, who cared for Mr Brown, said it had apologised for the delay in sharing the investigation report into his death.
It said it had implemented the recommendations of that report but said the coroner in the case had ruled that Mr Brown’s death was suicide and had not made specific criticisms of the care provided by the trust.
