Social care: Finding support in the community
Social care is a term which really refers to the broad range of things needed by some people. They don't have to be in hospital but they do need some help.
Funding for social care in England has been cut in recent years. There are delays and backlogs in hospitals meaning many older people can be stuck there because there is no help ready for them on the outside.
PM reporter Andrew Bomford finds out about a scheme in Derby that aims to keep people out of hospital and out of the care system.
