The government has announced new measures - aimed at reducing the number of deaths and serious injuries during childbirth in England.

The Health Secretary, Jeremy Hunt, says these will include an independent investigation into every case where something goes seriously wrong.

Jack Hawkins, whose baby was stillborn, told us coroners should investigate these cases, alongside independent reviews.
