We’ve had hundreds of nominations - and over the last eight weeks you've heard the stories behind the finalists.
For our final week, you'll meet the nominees in the carer category. This is awarded to someone who improves the life of an individual or group of people through their helpfulness, compassion and support.
Gerry Riley goes above and beyond caring for elderly people in her community, with no job being too big for her to help out with. She was nominated for this award by her son Richard, who Gerry nursed back to health after he had a stroke. Matt Smith went to meet them both.
