We’ve had hundreds of nominations - and over the last eight weeks you've heard the stories behind the finalists.
For our final week, you'll meet the nominees in the carer category. This is awarded to someone who improves the life of an individual or group of people through their helpfulness, compassion and support.
For 12 years of her life, Helen lived through Domestic Abuse. But having transformed her life with the help of Leicestershire charity Living Without Abuse, she now volunteers to help other survivors. She shared her story with Ady Dayman:
