More than three quarters of elderly patients referred to a specialised service from A&E go home on the same day. Yeovil Hospital Trust’s Frail Older Persons Assessment Service reduces pressure on A&E by assessing patients in a separate unit. Paul Mears, Chief Executive of Yeovil Hospital Trust, told 5 live: “Our aim is to make sure that our frail older people that come into hospital are given a comprehensive assessment but away from the busy Emergency Department.” The assessment unit is led by a team that includes a doctor, pharmacist and physiotherapist.
