Community Care for the elderly
Community Care allows an older person to remain as independent as possible while remaining in their own home. Each person is given an individual care plan tailored to their individual needs. This can include services such as home helps, doctor's visits, respite care and meals on wheels. For some elderly people, community care leads to a 'revolving door of care'.
